A person’s death must be reported by the family members from their household. 

If there are no such family members or they are incapable of doing so, a person's death must be reported by other persons who lived with the deceased or other family members upon learning about the person’s death, or the persons in whose apartment the deceased lived. 

If there are no such persons either, a person’s death must be reported by the first person to learn about it. 

For persons who die in a medical institution, while serving in the Croatian armed forces, in a correctional facility, a hotel, a boarding school or other institution, their death is to be reported by the competent institution or authority.   

Medical certificate of cause of death

A medical certificate of cause of death is a document issued by a physician or other person authorised to certify death, or the medical institution in which a person died. Persons reporting a person's death must present the death registration form and the medical certificate of cause of death.
